Position Description Empath Health is a nonprofit integrated network of care that provides expert, life-changing health care to those with chronic and advanced illnesses. Empath Health includes hospice, palliative home health and physician services, independent living assistance and day care for seniors, HIV services and advance care planning. Suncoast Hospice was established in 1977 by a group of volunteers who realized the value of hospice care and wanted to bring it to their community. As a member of Empath Health, we are a nonprofit organization that provides care, compassion and comfort to thousands of Pinellas County residents. Empath Health and its family of programs is looking for a patient-focused Physical Therapist to join our team. The Physical Therapist provides physical therapy for patients as required for diagnoses, age and treatment. Plans, organizes, directs and monitors patient care and mentors staff and students' clinical education. Supervises Physical Therapy Assistant. Essential functions and other responsibilities of this role include: --- Performs and documents evaluation for each patient conducting appropriate tests and measurements for age and diagnosis specific areas. --- Develops and implements individualized plan of care with established goals and outcomes and documents all visits per agency guidelines. Treatment plan is developed in accordance with patient and family wishes. --- Reviews and revises treatment plans and goals as needed by re-assessing the patient's status and response to treatment. --- Facilitates interdisciplinary patient care by collaborating with the patient/family, interdisciplinary team and other appropriate members of the health care team. --- Educates the patient and family by providing information and resources. --- Accepts personal responsibility for professional practice by adhering to state and federal regulations, Standards of Practice, and established policies and procedures. --- Arranges and initiates routinely co-treatment/supervisory time for physical therapy assistants --- Contributes to the prevention of infectious diseases among employees and patients by adhering to infection control policies and protocols. --- Performs other duties as requested by the Director Care Support Services within the scope of own professional license and in accordance with agency mission, vision and values. Position Requirements As a Physical Therapist with Empath Health you must patient/family-focused, motivated and interested in serving our community. Other requirements of this role include: --- Education and/or Experience: Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. --- Licensure: Licensed in the State of Florida, and meets one of the following requirements: o Graduated after successful completion of a physical therapist education program approved by one of the following: --- The Commission on Accreditation in Physical Therapy Education (CAPTE). --- Successor organizations of CAPTE. o An education program outside the United States determined to be substantially equivalent to physical therapist entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists. o Passed an examination for physical therapists approved by the state of Florida. --- Continuing education: 24 CEU's within the 2 year licensing period including medical errors. --- Certifications: CPR Our commitment to patient, client, staff and volunteer safety is a cornerstone of a High Reliability Organization with a focus on zero harm.
