Job Description:
This is a home health position for a Physical Therapist (PT).
The PT is responsible for creating the Plan of Care and
implementing interventions based on the POC and goals. PT will be
responsible for maintaining contact with the PTA and coordinating
care between patient and POC. The therapist will manage their own
schedule and drive to patients throughout the day. This job is very
autonomous, perfect for those who don't enjoy being kept working in
a facility all day.
